One year program

1. Submit the application form for Dr. Lazarus to review. Dr. Lazarus only accepts a small number of patients every year in order give personalized focus on each patient.

2. If UPROOTED is a good fit, we will set up your patient portal, take payment for membership and program, and sent out your initial questionnaires.

   Note: Prior to starting, you may be asked to have a full physical with you primary care or other physician.

3. You will fill out questionnaires and medical history in your portal.

4. Book your initial consultation with Dr. Lazarus. Dr. Lazarus will order your labs which will be mailed to you.

5. Complete the lab kits (may require a blood draw at a local facility or mobile phlebotomy - separate fee to facility may be needed).

6. You will then have follow-up virtual sessions with Dr. Lazarus where your labs will be reviewed, supplements and/or medications adjusted, and lifestyle recommendations given.

The art of “root cause” medicine is not adequately supported by health insurance companies or government sponsored programs. We do not accept Medicare, Medicaid, public or private insurance of any kind.

We will work with you if you have an take HSA, credit cards, debit cards, ACH and upon request will give you a “superbill”. Payment plans are also available.

While our services do not include traditional “primary care” services, Dr. Lazarus will personally design your health program providing individualized care. This rare and advanced program helps to address the root cause of your health concerns. UPROOTED’s services are complementary to the vital care that your primary care physician and traditional specialists provide.

-New patient virtual visit with Dr. Lazarus (45-60 minutes)

• (review of medical history, ordering of tests and lifestyle recommendations)

-Follow-up virtual visits with Dr. Lazarus (30-45 minutes) - every 3 months (3)

• (review follow-up questionnaires, lab results, lifestyle/supplement/medication adjustments)

• Health Insurance - we advise that you keep your primary care physician, specialist providers, and health insurance coverage to cover primary care, urgent care, emergency care, surgery, or any other medical need that arrises.

Membership is $45 per month ($540 yearly), this ensures your place with Dr. Lazarus and covers administrative fees

Adult - Body Restore Program

• Physician Services - $3000 - (can be paid in 4 installments of $750)

• Estimated labs over the course of a year ~$3000-$4000* in addition to the physician services fee

Children - Nutritional & Genetics Assessment Program

• Physician Services - $1400 - (can be paid in 4 installments of $350)

• Estimated labs over the course of a year ~$1000-$1200* in addition to the physician services fee

Prescriptions, supplements, lifestyle - varies per patient

*laboratory fees are subject to change and are at cost with laboratory or laboratory processing services

Yes, we can take things slower. Per physician service (consultation and/or review of findings) is $450 in addition to laboratory fees if needed.

Most functional labs are not adequately covered by insurance companies. However, preliminary screening labs may be done by your primary care physician before starting the program.

On a case by case basis, your physician services, laboratory fees, prescriptions, and supplements may be paid for using HSA fees. If you plan to use your HSA, please let us know so we may document accordingly.

It is vital that your primary care and specialists know what supplements and prescriptions you are taking. We do not automatically forward your medical records to your primary care physician. We ask that you communicate your labs/prescriptions/supplements to your primary care physician and specialists.

Complementary personalized root-cause medicine sometimes also refered to as functional medicine or in some cases holistic medicine has been in practice for decades, however not all traditional medical establishments are aware of all treatment practices. All of your medical records are available to you if you choose to forward them and would be happy to collaborate as needed.
